From Excel click...

QXL Stat Tools Tab > FMEA > Add > Failure Cause

Add Cause Dialog

Use this option to add Failure Cause(s) in selected Failure Mode.

  1. Number of Causes to add: Enter the number of failure causes that will be added to the current failure mode (must be an integer greater than 0).
